Use this form to report the transfer of Frozen Semen for Artificial Insemination to a new owner.

If applicable, add the mare to the Stallion Breeding Report and file a Breeding Certificate with the Office. 

Copies of the Frozen Semen Transfer Certificate and the Breeding Certificate should be retained by Stallion and Mare owners for reference when registering the resulting foal.

There is NO FEE for submitting this certificate to the Office.

 

The breeding agreement/contract is between the stallion and mare owners.

The Association needs to be informed of a stallion lease, date and manner a mare is bred to make registration a clean process.
